Carvistar CR 20 mg

Carvistar CR 20 mg is a controlled-release medicine containing 20 mg of carvedilol. Carvedilol blocks both beta and alpha receptors, helping slow the heartbeat and relax narrowed blood vessels.

The letters CR mean controlled release. The tablet releases carvedilol gradually instead of delivering the full dose at once. This allows it to provide a prolonged effect with once-daily administration.

Carvistar CR is used to manage high blood pressure, chronic heart failure and reduced heart-pumping function after a heart attack. It is also prescribed in certain treatment plans for stable angina because reducing the heart rate and workload decreases the heart’s demand for oxygen.

The controlled-release tablet should not be treated as interchangeable with the same strength of standard carvedilol. The formulations follow different dosing schedules and require a proper dose conversion.

Carvistar works on three important receptors in the heart and blood vessels.

By blocking beta-1 receptors, carvedilol slows the heart rate and reduces the force of each heartbeat. This means that the heart requires less energy and oxygen to pump blood.

Blocking alpha-1 receptors allows the blood vessels to relax and widen. This reduces resistance inside the arteries and lowers blood pressure.

Carvedilol also blocks beta-2 receptors, which is why the medicine requires special caution in people with asthma or other conditions that narrow the airways. The controlled-release formulation delivers the medicine gradually to provide an effect throughout the day.

Carvistar CR is generally taken once daily in the morning with food. The tablet should be swallowed whole and must not be crushed, chewed, broken or divided because this can interfere with its controlled-release action.

For hypertension, 20 mg once daily is the usual starting dose. If additional blood pressure control is needed and the medicine is well tolerated, the dose can be increased to 40 mg and then to a maximum of 80 mg once daily at intervals of one to two weeks.

For chronic heart failure, controlled-release carvedilol is generally started at 10 mg once daily. The dose can then be increased to 20 mg, 40 mg and 80 mg at intervals of at least two weeks, depending on heart rate, blood pressure, fluid retention and treatment response.

For stable patients with reduced heart function after a heart attack, treatment can begin at 20 mg once daily, followed by 40 mg and then 80 mg when tolerated. A lower starting dose is used when low blood pressure, slow heart rate or fluid retention is a concern.

Therefore, Carvistar CR 20 mg can be a starting dose for some conditions and a later titration dose for others. The strength should not be selected only from a blood pressure reading or changed without reviewing the full treatment plan.

Common Side Effects

Common side effects of Carvistar CR 20 mg include dizziness, tiredness, low blood pressure, a slower heartbeat, headache, diarrhoea, dry eyes and weakness. Some people can experience weight gain or fluid retention, particularly while treatment is being started or increased. Carvedilol can also affect blood glucose and may hide the fast heartbeat that normally warns of low blood sugar.

Serious Side Effects

Serious reactions require prompt medical attention. Warning signs include fainting, an unusually slow or irregular heartbeat, worsening breathlessness, rapid weight gain, swelling of the legs, wheezing, difficulty breathing, chest pain or swelling of the face, lips, tongue or throat. These symptoms can indicate severe bradycardia, heart block, worsening heart failure, bronchospasm or a serious allergic reaction.

Carvistar CR 20 mg should be avoided by:

• People allergic to carvedilol

• People with bronchial asthma

• People with severe bronchospasm

• People with second- or third-degree heart block

• People with sick sinus syndrome

• People with severe bradycardia

• Patients in cardiogenic shock

• Patients requiring intravenous heart-support medicines

• People with severe liver impairment

• Children unless instructed by a specialist

• Pregnant or breastfeeding women without medical assessment

A pacemaker can change whether carvedilol is suitable for someone with a heart-conduction condition. Diabetes, thyroid disease, kidney disease, poor circulation and a history of severe allergic reactions also require careful assessment.

Tell the healthcare provider about all medicines and supplements being used.

• Digoxin

• Amiodarone

• Verapamil

• Diltiazem

• Clonidine

Digoxin, amiodarone, verapamil and diltiazem can further slow the heartbeat or interfere with electrical conduction through the heart. Diabetes medicines can lower blood sugar more strongly, while carvedilol can hide warning signs such as a fast heartbeat. Some antidepressants can increase carvedilol levels, and rifampicin can reduce them.
