What Is Telmisartan?

Telmisartan is an angiotensin II receptor blocker (ARB) used primarily to treat high blood pressure and help reduce the risk of certain cardiovascular complications in appropriate patients. It works by blocking the action of angiotensin II, a hormone that causes blood vessels to narrow, thereby helping blood vessels relax and allowing blood pressure to decrease. Micardis is a well-known brand of telmisartan, and generic telmisartan tablet options are also available. Telmisartan helps control high blood pressure but does not cure the underlying condition, so it should be taken regularly as prescribed even when blood pressure is controlled or no symptoms are present.

The main telmisartan uses include:

• Reducing cardiovascular risk in certain high-risk adults who cannot take an ACE inhibitor, where approved

• Forming part of combination treatment when one medicine is insufficient

Specialists sometimes use ARBs for heart failure or diabetic kidney disease, but approved uses vary by country.

Benefits of Telmisartan

How Does Telmisartan Work?

Telmisartan works by blocking angiotensin II at AT1 receptors, preventing this hormone from causing blood vessels to narrow. Telmisartan belongs to the angiotensin II receptor blocker (ARB) class and is not an ACE inhibitor. Although both medicines act on the same renin-angiotensin system, they work at different stages of that pathway.

Telmisartan Dosage: 20 mg, 40 mg and 80 mg

The appropriate telmisartan dosage depends on the condition being treated, the patient's blood pressure, medical history, and other medicines being used. For hypertension, the usual labelled adult starting dose is telmisartan 40 mg once daily, although some patients may be prescribed 20 mg or have their dose adjusted up to telmisartan 80 mg once daily. In the United States, an 80 mg once-daily dose is also labelled for reducing cardiovascular risk in certain eligible adults. These strengths are not personal dosing instructions; liver impairment, dehydration, kidney function, other medicines, and the treatment goal can influence the appropriate regimen. Telmisartan is generally taken once daily at about the same time, with or without food. Do not increase, decrease, or repeat a dose independently.

Telmisartan Combination Tablets

Telmisartan and amlodipine combine an ARB with a calcium-channel blocker, while telmisartan and hydrochlorothiazide or telmisartan with chlorthalidone combine telmisartan with a diuretic. These combination medicines may be prescribed when blood pressure is not adequately controlled with a single medicine or when combination therapy is otherwise appropriate. Combination tablets are not automatically interchangeable with single-ingredient telmisartan tablets because the active ingredients and strengths differ. Amlodipine may cause effects such as ankle or peripheral swelling, while diuretics can affect fluid balance and electrolyte levels. Always check the active ingredients and strengths before switching between products.

Telmisartan Side Effects

Possible telmisartan side effects include dizziness, back pain, diarrhoea, headache, tiredness and sinus symptoms. Dizziness may increase after a dose change or during dehydration.

Seek urgent medical help for:

• Facial, lip, tongue or throat swelling or difficulty breathing

• Fainting or severe dizziness

• Reduced urination or swelling of the hands, feet or ankles

• Muscle weakness, tingling or an irregular heartbeat, which may suggest high potassium

• A severe or blistering skin reaction

Is Telmisartan Safe for the Kidneys and Liver?

Telmisartan can help protect kidney health in some people by improving long-term blood-pressure control, but it can also affect kidney function in susceptible patients. The risk may be higher during dehydration or when telmisartan is taken with medicines such as NSAID painkillers. Healthcare professionals may monitor creatinine and potassium levels, particularly after starting or changing treatment. Liver or bile-duct problems can increase exposure to telmisartan and may require a lower starting dose or closer medical monitoring.

Who Should Avoid Telmisartan?

Telmisartan must not be used during pregnancy because medicines that block the renin-angiotensin system can cause serious injury or death to the developing baby. Stop telmisartan and contact your prescriber promptly if pregnancy occurs. Aliskiren must not be combined with telmisartan in people with diabetes because of the increased risk of kidney problems, high potassium, and low blood pressure. Before treatment, tell your healthcare professional about allergies, kidney or liver disease, low blood pressure, dehydration, and breastfeeding so that the benefits and potential risks can be assessed.

Telmisartan Drug Interactions

Tell your prescriber or pharmacist about all medicines, vitamins, and supplements you use, especially:

• ACE inhibitors, other ARBs, or aliskiren

• Potassium supplements or potassium-containing salt substitutes

• Potassium-sparing diuretics and other diuretics

• NSAIDs, including ibuprofen or naproxen
